I was playing Wraith and bought Cheat Death. When I died, the item procced, but the effect immediately expired, and I died anyway.
The enemy team did not have Curse (or any effect that should have prevented Cheat Death from working). I was killed by a mine.

Yamato got the killing blow with bullets while bebop was beaming me with his ult.

Other team had Curse, but it was on cooldown according to the replay (could be bugged). Cheat death was also placed on cooldown.

Happened to me twice in one game. From a Seven and Ivy, one thing they had in common was that they both had lightning scroll. Tested it in the sandbox and it happened once more, but couldnt get it to happen again.
