Im pretty sure this is a bug and i have recreated it in sandbox.
Occurs when u have T3 Silver ult where it refreshes duration and CDs.
If you are hitting an enemy then paradox swaps them and you dont land another hit before they die you dont get the refresh.
Am assuming this is a bug but ya never know
